INSTRUCTIONS AND NOTES FOR MYNIC RESELLER APPLICATION FORM

Kindly follow the instructions below when filling out the application form:-

1. Application form must be as per template provided.

2. The maximum number of pages (excluding requested attachments) is six (6). The minimum font size is eleven (11).

3. Completed application forms (together with the attachments) may be forwarded by post, courier or by hand. In addition, please also e-mail a soft copy of the completed application form to reseller-app@mynic.net.my.

Please also note the following:-

1. Sections 2 and 4-9 of the application form are mandatory requirements.

2. MYNIC reserves the right to request for additional information from you to substantiate or support the application further or to clarify any matters.

3. MYNIC is not responsible for any expenses incurred which are incidental or which arise from the preparation of the application.

4. MYNIC reserves the right to reject the application and/or to suspend or cancel the Reseller Programme at any time without specifying any reason. MYNIC will be under no obligation to make any compensation or any other payment to the applicant or any other party.
